Coming from Phoenix

  1. 1. Follow I-10 W

  2. Use the right 2 lanes to take exit 124A to merge onto AZ-303 Loop N/Arizona State Rte 303 N

  3. Take exit 116 for Bell Rd

  4. Keep right at the fork and merge onto W Bell Rd. You will see Taco Bell to the right

  5. Follow W Bell Rd to the office

  6. Turn right at W Brookside Ln. You will see Adelante Healthcare

  7. Turn right

 

Coming from North Phoenix

  1. Follow AZ-101 Loop W and W Bell Rd to your destination in Surprise

  2. Turn left onto N 7th St

  3. Turn right onto E Beardsley Rd (signs for Arizona101 Loop W)

  4. Use the left 2 lanes to take the ramp onto AZ-101 Loop W

  5. Take exit 14 toward Bell Rd

  6. Keep right at the fork. Follow signs for Surprise Stadium

  7. Merge onto W Bell Rd. Pass by Whataburger

  8. Turn left at W Brookside Ln. You will see Adelante Healthcare to the left

  9. Turn right

 

Coming from Wickenburg

  1. Take N Valentine St to E Center/E Wickenburg Way

  2. Head east on E Apache St toward N Valentine St

  3. Turn right at the 1st cross street onto N Valentine St

  4. Take US-60 E to W Bell Rd in Surprise. Take exit 116 from AZ-303 Loop S/Arizona State Rte 303 S

  5. Follow W Bell Rd to your destination

  6. Use any lane to turn slightly left onto W Bell Rd. You will see Winco Foods and Starbucks to the right

  7. Turn right at W Brookside Ln. You will see Adelante Healthcare to the right

  8. Turn right

 

Coming from Buckeye

  1. Follow 303 North

  2. Take exit 116 for Bell Rd. You will see a Taco Bell to the right

  3. Keep right at the fork to continue toward W. Bell

  4. Turn right at W. Brookside Ln. You will see Adelante Healthcare to the right

  5. Turn right
